**$5,000 - Sign-On Bonus for Full-time externals only, pro-rated for Part-time**

Full time Nurse Navigator position in a Bariatric/Metabolic/General Surgery office in Rhinebeck, NY. 40 hours per week, 8 hour shifts 8:00am-4:30pm.

 

Summary: The Bariatric Surgery Nurse Navigator facilitates patient navigation to bariatric surgery during the pre-op, intra-op and post- op phases according to the standardized bariatric surgery patient clinical care pathway. The Bariatric Nurse Navigator will serve as a dedicated point of contact for patients throughout their weight management journey. This role involves coordinating care, providing education, and offering emotional support to patients considering or undergoing bariatric surgery.

 

Responsibilities:

  1. Provide comprehensive education to patients about bariatric surgery, weight loss options, post-operative care, and lifestyle changes.
  2. Explain preoperative preparation, surgical procedures, and potential risks to patients and their families.
  3. Offer emotional support to patients throughout their weight management journey.
  4. Coordinate the scheduling of appointments, preoperative testing, and communication/consultations with other specialists.
  5. Act as the primary point of contact for patients, addressing any questions or concerns throughout the process.
  6. Follow up with patients to ensure they are adhering to pre-surgical and post-surgical instructions.
  7. Perform assessments of patients’ health status and provide recommendations based on programmatic and insurance guidelines.
  8. Ensure that patients meet the required criteria for bariatric surgery, including completing necessary evaluations and screenings.
  9. Collaborate with the surgical team, dietitians, behavioral health specialists, and other healthcare professionals to ensure holistic care for bariatric patients.
  10. Participate in team meetings to discuss patient progress and any issues that may arise during the patient’s care process.
  11. Identify and address any barriers to care that may affect patient outcomes or experience.
  12. Participate in quality assurance and performance improvement initiatives related to bariatric care.
  13. Will perform other duties as assigned, including but not limited to providing direct patient care in the office setting
Required: NYS RN License, BSN, minimum 3 years nursing experience in a med/surg setting 
Experience: 

Minimum of 3 years of nursing experience, preferably in bariatrics, weight management, or a related field.

Experience in outpatient care settings is highly preferred.

Experience in patient care navigation is highly preferred.

Skills & Abilities: 

Strong knowledge of bariatric surgery options, preoperative and postoperative care, and associated health risks.

Excellent communication and interpersonal skills with the ability to engage with diverse patient populations.

Ability to work independently and as part of a multidisciplinary team.

Strong organizational skills and attention to detail.

Proficient in the use of Electronic Health Records (EHR) and other healthcare software.
