Cardiology Medical Office and Heart Failure RN position in Poughkeepsie, NY.
Full time, 40 hours per week, Monday-Friday between 8:30am-5:00pm.

 

Summary:

Heart Failure nurse serves as a resource for patients, providers, staff performing in the role of educator, direct patient care and quality improvement for outpatient/inpatient support for patients in the Congestive Heart Failure (CHF) program. Duties include but are not limited to assessment, implementation, evaluation related to discharge education, collection of data, assistance in treatment planning for self-management as well as follow-up care to reinforce adherence with the treatment plan, responding to changes in condition to decrease risk of readmission.  Population served based on scope of services in the department.                    

Responsibilities:

 

  • Works with all clinical teams as a resource on care management of all practice patients to include the following: pre-visit planning workflow to ensure care completion prior to visit, and after visit review with patients.  Assists with record keeping/data collection for HF consults and follow-up care.
  • Collaborates with physician leadership to define quality measure/outcome reporting process and develops protocols and point of care reminders using nationally recognized measure/outcomes.   Serves as a resource to clinical staff and providers to establish quality goals using reports.
  • Manages assigned panel of high acuity chronic care patients, primarily HF patients. Performs Heart Failure Education and supports HF Patient Support group; Referrals to LTC, hospice, palliative care, and home care and explanations/educational resources regarding orders, medications, and tests/procedures (i.e. Cath, MRI, echo, cardiomems implant, BI-V/AICD etc. 
  • Provides ongoing and frequent follow up contact with patient to ensure compliance with recommendations for self-care.  Tracks patients follow up; i.e.no shows daily and ensures rescheduled appointment; hospital follow-up compliance; Calls to no-show patients for management and rescheduling; coordinates care with rehab, nursing homes, etc.; arranging transportation if needed, etc.   
  • Provides telephone advice according to protocol and handle urgent and emergent calls for all patients, include HVCP triage needs per HVCP clinical protocols.  Maintains close contact with patients discharged, via phone, for 30 days. (weekly phone call as needed) 
  • Responsible for working with the patients' care team to coordinate change readiness, needs assessment, and develop an individualized treatment care plan. Completes necessary documentation and pre-visit planning that is requested before visit.
  • Assists patients in setting goals for self-management tasks and reports abnormal findings to their physician tea, including development, procurement, and adoption of patient self-management educational resources used by the primary clinical teams.  
  • In-office administration and documentation of diuretics such as Lasix and other medications as needed. 
  • Collaborates with payer Case Managers for additional services when appropriate.
  • Cross trained to cover HVCP triage as needed.
  • Maintains workbooks of patient interactions for CHF patients. Will also maintain and act upon a list of those patients using Cardiomems devices.
  • Maintains and Models Nuvance Health Values. Fulfills all compliance responsibilities related to the position.
  • Demonstrates regular, reliable and predictable attendance.
  • Performs other duties as required.
Education and Experience Requirements:
  • Minimum two (3) years’ experience Acute Care nursing or equivalent experience,
  • Associates Degree in Nursing required - Bachelor’s Degree in Nursing preferred.
 
Minimum Knowledge, Skills and Abilities Requirements:
  • Computer Skills/data entry, Microsoft WORD, Excel, Outlook, Presentation skills, Verbal and written English proficiency, etc.
  • IV Insertion skills
 
License, Registration, or Certification Requirements:
  • NYS RN License, ACLS, BLS
