JOB SUMMARY: Performs a variety of clerical and computer related activities and phlebotomy with minimal supervision. This individual interacts with patients of all ages and works effectively with others to meet the needs of the patients.
JOB QUALIFICATIONS
Education: High School diploma or equivalent is required.
Experience: Prefer six months of phlebotomy experience, preferably in a hospital setting. Previous computer experience is preferred. Effective interpersonal skills are also required.

RESPONSIBILITIES AND STANDARDS
Responsibility I
1. Performs Phlebotomy activities on patients of all ages with minimal supervision.
1.1 Collects Stat, Timed and other samples by venipuncture or capillary puncture, being aware of the importance of timing in instances such as therapeutic drug levels, etc.; follow established guidelines for laboratory specimens.
1.2 Verify all patient information prior to specimen collection.
1.3 Label all samples collected. (Bar code labels).
1.4 Initial, Time and Date all blood specimen tubes. (Bar code labels).
1.5 Maintains specimen integrity by using aseptic technique, following departmental procedure and observing isolation procedures.
1.6 Performs other duties such as data collecting, filing, faxing, assisting technologist and hospital wide laboratory courier for sample pick-up and transport to the laboratory.
1.7 Maintains copies of outpatient orders in accordance with regulatory requirements.
1.8 Maintains copies of manually requested patient tests in accordance with regulatory requirements.
1.9 Exhibits polite, considerate and caring attitude when interacting with patients, their families, physicians and hospital staff.
1.10 Reports system problems to the Hospital Information System (HIS) Help desk.
Responsibility II
2. Specimen Processing of all samples submitted to laboratory for testing. Provides assistance to all incoming phone calls to the lab.
2.1 Verifies all samples are labeled correctly.
2.2 Verifies all samples are in correct containers/ tubes/ media.
2.3 Receives all samples in LIS.
2.4 Reprints labels as needed for sample testing.
2.5 Centrifugation of samples.
2.6 Visually verify all sample integrity prior to delivery to testing departments. If samples are adequate, deliver to departments for testing. If samples are inadequate, follow established Cancellation Policy.
2.7 Provides accurate information and timely resolution to physicians/nursing personnel who call lab.
2.8 Answers all telephone calls in a courteous manner and routes to the appropriate individual.
2.9 Records messages accurately and routes to appropriate individual.
2.10 Retrieves patient laboratory test data using computer functions.
2.11 Vocera: Carries and responds to Vocera communication device.
2.12 Vocera is checked at the beginning of each shift for battery life and functionality prior to use.
2.13 Vocera sign-IN and sign-OUT sheets are signed each and every day of duty.
2.14 It is the responsibility of the phlebotomist to sign-in and sign-out the Vocera. Failure to do so will result in corrective action given. Vocera becomes lost, the phlebotomist who last used the device will receive corrective action.
Responsibility III
3. Performs Computer related activities as required.
3.1 Monitor patient collection pending log each hour.
3.2 Maintain accuracy of pending log, sensor for duplicate orders or unusual test orders.
3.3 Receive samples in LIS.
3.4 Reprint labels if necessary.
3.5 Test ordering for miscellaneous samples.
3.6 Test ordering for Send-Out/ reference laboratory samples. Generating reference laboratory manifest and packaging of all samples for courier pick-up.
3.7 Competency with use of laboratory hand-held equipment. (Scanner and label printer).
3.8 Competency with all LIS systems.
Responsibility IV
4. Advises Manager of complaints and problems.
4.1 Reports possible safety hazards to the Laboratory Supervisor immediately.
4.2 Reports any and all discrepancies in relation to patient care to the Laboratory Supervisor or designated In-Charge authority. (Wrong patient collected/ sample mislabeling, acts of violence or altercations, etc.).
4.3 Reports any and all on the job accidents immediately. (Needles stick, slip and falls, etc..).
4.4 Offers solutions to problems or issues that may impact laboratory effectiveness on patient care.
4.5 All reports of discrepancies must be documented in writing and given to the department supervisor for immediate action.
Responsibility V
5. Maintains professional skills and competencies.
5.1 Performs required testing for competency evaluation.
5.2 Completes all mandated competencies for position.
5.3 Completes hospital required mandatory education.
5.4 Accepts special project assignments and completes by the deadline date.
5.5 Performs new procedures with little difficulty after training.
5.6 Presents to staff knowledge gained through participation at continuing education activities.
5.7 Develops and reviews professional goals in conjunction with department supervisor.
5.8 Continuously develops skills necessary to improve job performance.
5.9 Assumes responsibility for self-growth.
5.10 Maintains current knowledge of laboratory procedures.
5.11 Participates in the orientation/training of new personnel.
5.12 Offers plans / suggestions for department improvement.
Responsibility VI
6. Follows hospital and departmental policies and procedures.
6.1 Adapts to changes in practice; accepting and implementing approved changes and learning new tasks.
6.2 Follows established laboratory safety and universal precautions practices at all times.
6.3 Maintains a clean, organized work area and keeps working supplies stocked at bench.
6.4 Attends department meetings as required or reads and initials meeting minutes.
6.5 Maintains awareness of changes in procedures and protocols.
6.6 Uses spare time in work-related activities contributing to lab needs.
6.7 Complies with hospital and departmental policies and procedures.
6.8 Arrives at work on time, begins work promptly and is available until end of shift.
6.9 Treats all patient information, including test results, confidentially; does not talk indiscreetly about test results, patients or coworkers.
6.10 Exhibits ethical work behavior.
6.11 Takes part in the quality assurance program of the laboratory.
6.12 Promotes a professional image of the laboratory/hospital by maintaining professional appearance and behavior.
6.13 Makes constructive suggestions for departmental improvements.
Responsibility VII
7. Demonstrates regard for the dignity and respect of all patients, family members, visitors and Medical Center personnel as identified in the philosophy of Health-Quest.
7.1 Displays a caring and courteous attitude and represents Health-Quest in a positive manner to all persons noted above.
7.2 Promotes and contributes positively to intra-departmental and inter-departmental relationships with no more than two complaints in a six-month period.
7.3 Maintains the confidentiality of employees and departmental information with no infractions.
7.4 Adheres to departmental and Health-Quest dress codes as observed by Supervisor, and wears a Health-Quest picture identification badge, 100% of the time.
