$30,000 Sign On Bonus! 
Purpose: The clinical manager is responsible for the oversight of all patient care services and personnel to ensure the delivery of quality care, communication/coordination of services, and integration of physician’s orders for the care and services provided to HQHC patients. Essential

Responsibilities:

  • 1. Oversees the daily home care operations from referrals, SOC, ROC, transfers, and discharges to accurately collect and document required patient information.
  • 2. Ensure the provision of quality care to patient's goals and needs through oversight of employee nursing services in consultation with physicians or by prescription.
  • 3. Serves as a resource to agency staff in the planning and provision of high quality patient care. Assists professional staff to problem solve complex patient problems through case conferences, education, and home tracer visits.
  • 4. Monitors and reports on standards of employee behavior, performance, proactively addresses employees who fail to meet performance standards, and shares with the Director of Patient Care Services.
  • 5. Performs utilization and quality assessment reviews monthly to maintain compliance with Federal and State regulatory requirements that includes record review, timely completion of MIDAs reports, transfers, discharges, F2F and OASIS documentation.
  • 6. Manages both on-call and regular employee schedules which includes all PTO requests with coordination of daily and weekend assignments to ensure that patient care needs are met and the plan of care is followed. Coordinates staffing coverage and communicates all changes to the patient, caregiver and/or representative.
  • 7. Conducts employee performance appraisals within agency guidelines and provides on-going feedback quarterly.
  • 8. Serves on HQHC Quality Assessment Performance Improvement (QAPI) committee and other committees as needed.
  • 9. Exemplifies excellent customer relations with patients, caregivers, physicians, nursing, therapy staff and serves as a positive role model for the department through initiative, involvement, and availability.
Education and Experience Requirements:
  • 1. Minimum of three to five years progressive professional experience, preferred with at least one year of full-time managerial experience in a community health or home care setting required.
  • 2. Acceptance of philosophy and goals of this Agency.
  • 3. Ability to exercise initiative and independent judgment.
  • 4. Demonstrates leadership behaviors evidenced with good verbal and written communication with organizational skills to manage and lead employees.

 Minimum Knowledge, Skills and Abilities Requirements: · Basic Computer EMR skills · NYS Driver’s License · Current Car Insurance Policy License, Registration, or Certification Requirements: 1. Bachelor’s degree in nursing required. Graduate degree in public health or health-related field preferred. 2. License and current registration to practice as a Registered Nurse in New York State required. 3. BLS CPR Certification. Environmental Factors: Factors affecting environment conditions may vary depending on the assigned work area and tasks. Potential environmental exposures include, but are not limited to: Works indoors in Agency office and patient homes and travels to/from patient homes
