Description

Summary:

 

Plans, develops, coordinates, and directs the operations of the Pathway to Excellence and/or Magnet program utilizing the American Nurse Credentialing Center. Oversees the execution of process and adherence to evidenced based protocols and develops performance improvement initiatives for the program. Assumes responsibility for program quality, compliance, and education of nursing staff related to evidenced based practice and shared governance. Serves as a clinical and educational consultant to the nursing staff, leadership, patients, and other members of the health care team related to the program. Coordinates and responsible for project management, editing and submission of Pathway document to ANCC and ensures all ANCC standards are met.

Responsibilities:

 

1-Leads the organization�s efforts to achieve and sustain ANCC Pathway to Excellence and/or Magnet designation. Oversees and coordinates site visit readiness and activities for ANCC Pathway to Excellence and/or Magnet. (20%)

2- Promotes departmental strategic planning that addresses implementation of standards and assists with integrating standards into daily nursing practice and organizational culture. (10%)

3- Facilitates periodic self-assessment / gap analysis and improvement planning to strengthen integration of standards. (5%)

4- Stays apprised of the American Nurse�s Credentialing Center�s (ANCC) revisions and clarifications regarding standards and communicates changes. Serves as the primary contact with ANCC, ensuring all communications and submissions are timely and accurate. (5%)

5- Develops and coordinates Pathway-related educational materials and presentations for all employees including providers and integrates Pathway to Excellence and/or Magnet standards into daily nursing practice and organizational culture. (10%)

6- Provides ongoing education about the Pathway and/or Magnet standards and principles of a professional practice environment to support the goals of Norwalk�s strategic plan. (10%)

7- Works closely with Communications and Marketing to produce ANCC-related publicity/advertisement materials and coordinates celebrations and displays (5%)

8- Chairs and coordinates meetings and activities of the Pathway to Excellence and/or Magnet Team and provides periodic updates to Nursing Leadership team ensuring that a culture of shared governance - at the unit and hospital level - and collaborative decision-making exists among nursing staff and leadership. (20%)

9 - Collaborates with leadership and quality on trending and performance improvement plans related to nursing quality indicators as it pertains to the ANCC Pathway to Excellence and/or Magnet Program.

10 - Ensures timely compliance for submission of all materials required for application / re-application and ensures compliance with ANCC Pathway to Excellence and/or Magnet standards are met. Remains current with changes in ANCC requirements and best practices in nursing. (15%)

11 - Collaborates with other hospitals and system Pathway to Excellence and/or Magnet Coordinators to identify best practices and promote shared success.

12- Maintains and Models Nuvance Health Values.

13. Demonstrates regular, reliable and predictable attendance.

14. Performs other duties as required.

 
Education/Licensure/Qualifications
  • Graduate of an accredited school of nursing; Master’s degree in nursing (MSN) required,
  • ANCC certification in a relevant specialty is desired.
  • Minimum of 3-5 years of experience in a clinical nursing role, with demonstrated leadership and project management skills.
  • Experience with ANCC Pathway to Excellence or Magnet is highly desirable.
