$40,000 SIGN ON BONUS - for eligible external candidates only*
Nuvance Health has a network of convenient hospital and outpatient locations — Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York — plus multiple primary and specialty care physician practices locations, including The Heart Center, a leading provider of cardiology care, and two urgent care offices.  Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.
Job Summary: Responsible for the evaluation, planning, directing and administering of occupational therapy modalities of treatment as prescribed by a licensed physician. Administers treatments and occupational agents as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease or physical disability. Assists patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities.
1. Assess the functional and therapeutic needs of the patient 
2. Properly documents of activity assessments 
3. Conducts group and individual therapies in accordance with patient diagnosis and needs. 
4. Conducts therapies in accordance with patient diagnosis and needs 
5. Completes progress notes and assessments according to program procedures. 
6 Is an active member of the interdisciplinary team; reports patient response to interventions at physician led team meetings. 
7. Contributes to the development of individualized treatment plans for each patient, auditing treatment plans for thoroughness. 
8. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. 
9. Plans and implements the unit’s activity therapy program, including planning of daily events, special events, and providing in-service for other staff participating in the activity
therapy program. 
10. Conducts activity therapy group sessions, as assigned, displaying good theoretical knowledge of group principles and good group technique. 
11. Identifies areas needing improvement and utilizes the facility performance improvement process. Actively participates in department processes as required. 
12. Conducts individual activity therapy sessions, as assigned, displaying good theoretical knowledge and good treatment technique 
13. Maintains records pertinent to the personnel and operation of the department.
Regulatory Requirements:
· Graduated from either a Masters of Occupational Therapy, Recreation Therapy, or other human service master’s degree program with an accompanying license for art, recreation or activity therapy OR Bachelor's in related field including  CTRS Certification Required.
· Three (3) or more years of previous experience in a behavioral health setting.
Language Skills:
· Ability to read and communicate effectively in English.
· Additional languages preferred.
· Strong written and verbal skills.
Skills:
· Basic computer knowledge.
· Knowledge of physical medicine, physical therapy modalities, anatomy and physiology.
· Knowledge of physical and occupational therapy machine operation, maintenance and repairs of same.

    Work Shift: 8am - 4:30pm, .  Every Sunday and the candidate will have off Monday.  The schedule will be Sunday, Tuesday, Wednesday, Thursday and Friday
