Summary:
The purpose of this job is to ensure that safe, efficient and optimal outcomes are achieved for patients planning surgical procedures. This job requires the implementation of evidence based practice protocols to ensure optimal outcomes for patients. It contributes to the organization's objectives by providing preoperative care that is patient centered, fiscally responsible and time effective. The purpose of this job is also to design and implement pain management plans for patients with both acute and chronic pain problems. This job is responsible for collaborating with the Director of the GPS Home in order to implement current Perioperative Surgical Home guidelines and recommendations per ASA guidelines.
Responsibilities:
1. Provides pre-admission assessment for patients to ensure that patients are optimized for surgery and ensures appropriate service line protocols are implemented. Collaborates with Director of GPS Home to determine need for further consultation and testing to ensure patient optimization. 25%
2. Plans and implements preoperative pain management plans for all chronic pain patients seen in PAT. Collaborates with Director of Pain Service to ensure continuity of care for patient seen in the GPS Home.
3. Plans and implements orientation and education for providers and staff, including RNs, APRNs, and Anesthesia Residents. Please see attached educational plan for example.
4. Maintains data base of PAT volumes and outcomes. Communicates relevant information to Department Chair. Develops and implements plans for optimizing volumes and promoting patient satisfaction.
5. Serves as a resource and provider to physicians, staff and patients to resolve issues related to pre-admission testing and evaluation, pain management and other identified anesthesia issues.
6. Assists the Director and Chair in new program development, market assessment and public relations, and assists with coordination and implementation of new projects and initiatives such as Northwell, Marshall Steele, and other innovations.
7. Collaborates with interdisciplinary team members to optimize patient outcomes by communicating relevant and critical information to avoid cancellations and delays of surgical procedures.
8. Responsible for scheduling APRNs to meet the needs of patients and the department.
9. Fulfills all compliance responsibilities related to the position.
10. Performs other duties as assigned.
Other information:
Required: Advanced Physical Assessment, Interpretation of Diagnostic Tests, Advanced Pharmacology, Excellent written and verbal communication skills, Competency in various computer programs Word, Excel, Electronic Medical Records.
Minimum Experience: five years
Desired: Effective teaching skills. Experience in developing and implementing education plans. Highly developed interpersonal communications skills including verbal and written skills.
