Description

Summary:

Provides behavioral health consultation to adult patients while in the acute inpatient rehabilitation unit. They work to strengthen patients coping skills, support successful adaptation and recovery, and facilitate the appropriate and effective management of emotionally related issues to move the rehabilitation process forward. They provide assessment and formulate treatment and discharge/transition plans in collaboration with the patient, family members and the multidisciplinary healthcare team. ,

Responsibilities:

1. Provides behavioral health assessments and treatment; provides consultation and support to patients families and significant others.;

2. Provides support to the health care team regarding management of  patients complex psychosocial issues.;

3. Provides individual and family, patient-centered support, and psychoeducation.;

4. Establishes objective s patient-focused treatment plan to provide disposition and/or referral. Identifies potential barriers, limitations & coping abilities, adjustments, and functional status.;

5. Assesses  patients  clinical readiness for transition in collaboration with the healthcare team and informs the team of the available resources based on the  patients  clinical needs. Maintains easy flow of communication regarding  patients  development and progress to all involved in the care of the patient.;

6. Maintains knowledge of community resources and refers patients & families to agencies for supportive services & concrete resources.;

7. May attend interdisciplinary team meetings to provide behavioral health perspective and input on patient care and treatment planning.;

8. In accordance with all regulatory requirements, documents accurately, objectively, and timely, the patients plan of care and treatment modalities and interventions.;

9. Collaborates effectively with team members, develops good working relationships both inside and outside of the team.;

10. Fulfills all compliance responsibilities related to the position.;

11. Performs other duties as assigned.;,

Other Information:

Education Skills Experience:;

The position requires CT licensure as a Licensed professional counselor (LPC) or Licensed Clinical Social Worker (LCSW) ; Graduate degree in clinical mental health counseling as part of a program of higher learning from an accredited school

Prefer Previous experience in physical rehabilitative setting or related setting.;

Minimum of 3- 5 years of experience preferred;

Working Conditions:

Manual: Some manual skills/motor coord & finger dexterity

Occupational: Some occupational risk

Physical Effort: Medium to Heavy effort. May exert up to 35 lbs. force

Physical Environment: Some exposure to dirt, odors, noise, human waste, etc.
