***$4,000 - Sign-On Bonus for Full-time externals only, pro-rated for Part-time***
Medical Office LPN Surgical Services department- Poughkeepsie, NY 40 hours per week, Monday-Friday 8:00am-4:30pm
 

Summary:

Performs tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a registered nurse or licensed health care provider.

Responsibilities:

  1. Greet patients in waiting area, escort patients to clinical areas, check-outs and answer patient questions.
  2. Telephone call-backs, and telephone care as directed by physician/provider and RN.
  3. Patient counseling and education as directed by provider (verbal, brochures, handouts).
  4. Responsible for HPN and NYSIIS immunization registration reprinting and account updates if applicable.
  5. Preparation of medical record in advance of appointment. This includes anticipatory tasks such as consent forms, ABNs, PHI Release, obtaining medical records from other providers/facilities, and updating of EMR, when appropriate.
  6. Patient check-in: obtain and document chief complaint, vital signs, history, ordered CLIA waived tests; Capture charges for provision of ancillary services.
  7. Assists patients with the completion of patient summaries, medication/allergy reconciliation at every office visit, history forms, and in obtaining appropriate treatment consents, including administering  and documenting  injections, dispensing and documenting medications as directed by physician/provider.
  8. Prepare patient and exam room; chaperone and assist physician/provider with examination, procedures and treatments if needed
  9. Labeling and handling of labs, pap smears, cultures, biopsies and other specimens; Preparation of laboratory and imaging requisitions.
  10. Assist with coordination of further patient care and follow-up, and carry out tasks as directed by RN and/or physician/provider. This may include communication with patient, hospital, labs, imaging centers, pharmacies, physician offices, insurance plans.
  11. Document all activities/interventions, patient/family responses, tests and treatments, in the medical.
  12. Assumes responsibility for patient flow and ensure that patients have follow up testing and appointment with physicians.
  13. Maintain and Model Nuvance Health Values.

  16. Performs other duties as required. 

Other Information:

Required:
  • Basic Life Support (BLS) certification within 30 days of starting.
  • Cardiopulmonary Resuscitation (CPR) Certification 
  • Licensed Practical Nurse licensure.
  • Basic medical knowledge and terminology.
  • Computer skills/data entry.
  • Skilled in obtaining accurate patient vitals.
  • Vaccine administration and medication preparation.
Preferred:
  • Minimum Experience: one year.
  • Proficiency in multiple languages.
  • Phlebotomy.
  • EKG.
Education Required:
  • HS Graduate or Equivalent.
  • Graduate of an accredited LPN Program.

Working Conditions:

Manual: significant manual skills/motor coord & finger dexterity

Occupational: Some occupational risk

Physical Effort: Very Heavy effort. May exert up to 50 lbs. force

Physical Environment: Some exposure to dirt, odors, noise, human waste, etc.
