Full-Time LCSW Womens Health 
Nuvance Health is seeking a full time Licensed Clinical Social Worker in Poughkeepsie, NY. This is a Monday through Friday position with no weekends. This position identifies high risk patients from a psychosocial/financial perspective, assesses the psychological needs of patients and families and provides information, support, counseling, care management, and referrals to appropriate resources. Resolves psychosocial patient care issues as it relates to women's health and develops and implements a complex patient transition/discharge plan.

Qualifications:
  • Master's degree in Social Work from an accredited school
  • a Minimum of 1 years’ experience
  • Current CT Licensed Clinical Social Worker (LCSW) in CT
